Acacia Network is now offering the COVID-19 vaccine at our designated Federally Qualified Health Centers (FQHCs) to pre-registered New Yorkers who meet the City’s eligibility criteria.
We are currently administering vaccines Monday-Friday from 11:00AM – 4:00PM, by appointment only. We have bilingual staff available to answer questions. All eligible individuals are welcome to make an appointment today!
Who is Eligible?
New York City has issued week-by-week eligibility and distribution guidelines. The COVID-19 vaccine is currently available for eligible individuals who meet NYC criteria, including Federally Qualified Health Center (FQHC) staff and those who provide direct in-person services within clinical and/or congregate care settings, individuals ages 65+ as well as those with underlying health conditions, and more. To find out if you qualify, visit: https://nyc.gov/covidvaccine
Where Can I Get Vaccinated?
IMPORTANT NOTICE: Due to high demand and a COVID-19 vaccine shortage across New York State, we are currently at capacity and do not have availability for new appointments. Please know that we are working with our City and State partners to ensure additional vaccine supplies to meet the needs of our communities. We encourage you to check this page often for updates.
