Acacia Network is now offering the COVID-19 vaccine at our designated Federally Qualified Health Centers (FQHCs) to pre-registered New Yorkers who meet the City’s eligibility criteria.
We are currently administering vaccines Monday-Friday from 11:00AM – 4:00PM, by appointment only. We have bilingual staff available to answer questions. All eligible individuals are welcome to make an appointment today!
Who is Eligible?
New York City has issued week-by-week eligibility and distribution guidelines. The COVID-19 vaccine is currently available for eligible individuals who meet NYC criteria, including Federally Qualified Health Center (FQHC) staff and those who provide direct in-person services within clinical and/or congregate care settings, eligible seniors, and more. IMPORTANT NOTICE: Due to high demand and a COVID-19 vaccine shortage across New York State, we are currently at capacity and do not have availability for new appointments. How Do I Make an Appointment?
- Determine whether you meet eligibility criteria. Request an appointment online at your preferred location:

***IF YOU ARE A SENIOR 65+: If you are an older adult 65 years of age and above, you can make an appointment by calling (718)734-2539.
2. Under “Referral Source”, select “Self-Referral” and complete the required contact information. Click “Continue”.
3. Select “Acacia Network” as the “Receiving Provider”. Attest that you qualify, choose appointment times, and click “Send Referral”.
Please note: Once an appointment has been scheduled, you will receive a confirmation and reminders. It is extremely important that you keep your appointment, as the vaccine has a short shelf life once it has been opened. If you must cancel or reschedule, please let us know as soon as possible.
